WHY REPORT SUSPECTED SIDE EFFECTS?
Making medicines safer for everyone
Your report is important and can make a difference. Side effects are undesirable effects, which may occur after taking a medicine and are not necessarily caused by the medicine. It is important to document and analyse side effects to find out, if the medicine has caused them. All medicines are capable of causing side effects in some patients. Many side effects are mild, but some can be serious and even life threatening. Occasionally, side effects may only occur after a person has stopped taking a medicine.
Some side effects occur (very) rarely or only after long-term treatment. Thus, they only become known once many people have been taking the medicine and/or they were taken for a long time. That is why it is important that you tell us about suspected side effects.
Sometimes it is hard to tell whether a symptom is a side effect from taking your medicine, or from something else. If you are unsure, please report the symptom and other details to us by using the form we provide.
WHAT INFORMATION DO WE NEED TO EVALUATE THE SUSPECTED SIDE EFFECT?

Information on the suspected side effect
Provide as much information as you can. You can also tell us if the harm was because of a mistake in how the medicine was administered – this is called a medication error. For example: wrong drug, dose or administration route; and why (such as, too much medicine taken because of wrong instructions on the package leaflet etc.). - Information about the patient who experienced the suspected side effect
Provide as much information as you can, such as age, sex, height and weight. This helps us differentiate between patients in our database. It also helps us understand if certain patients or age groups are more vulnerable to experiencing a side effect.
- Information on the medicines, which might have caused the suspected side effect
If available, please also provide the batch number (BN) which you can find on the medicines box. This is very important for drug product produced from biological sources (such as blood plasma).
Sometimes side effects can also happen because of interactions, so we request that you to tell us about all medicines or herbal remedies which have been taken by the patient.

We need a name and contact details as it helps us to get in touch with you if we need some clarification on the information provided.
